101229(a)(1) · 101229 Responsibility for Providing Care and Supervision (a)(1) (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.

Director will provide training to staff and provvde a statement to LPA on how the the facility will ensure children have supervision at all times. Statement will be provided to LPA via email at giovani.gonzalez@dss.ca.gov no later than 10/24/25. Based on interviews conducted, and record review, Licensee did not comply with the deficiency cited above in leaving children unattended during nap time, which poses an a potential risk to the health, safety and or personal rights of children in care.

101223(a)(1) · 101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights (1) To be accorded dignity in his/her personal relationships with staff and other persons. Facility will continue to work with Holly Harven from inclusion program to help staff reduce biting incidents and provide a written statement stating how the facility will prevent biting from happening less frequently in the future and include behavior plan(s) for the children that are biting.
