This will be submitted to LPA George via email @ elizabeth.george@dss.ca.gov no later than October 17,l 2025. Upon review of the infant sleep logs report from bright wheel, staff documention of 15 minute checks from the time the child falls asleep till the time the child is awake from their naps is not completed with some days no documentation of 15 minutes sleep checks at all.

Facility will write a letter to the deparment, the letter will confirm facility understands the aformentioned regulation, if the facility does not understand the regulation, Director will contact LPA Negrete. Based on LPA's observations and interviews conducted, it is evident that the facility was not in compliance due 1 teacher and 1 teacher aide supervising 9 infants(not asleep). The facility will write a separate letter. The letter will address the reason why Section 101216(b) should or should not be implemented at this facility by the Department."

Based on observation and document review Facility will draft a memo for parents. The memo will be read and signed by every parent, in order to remind parents of Title 22 CCR, Section 101229.1(b). The Center will submit a copy of the signed memo via email to german.negrete@dss.ca.gov the Center did not comply with the section cited above, on 7/17/2024 C31 signed in at 9:14AM, no sign out was registered. 101438.2(d) · 101438.2(d) Outdoor Activity Space for Infants: The outdoor activity space shall be equipped with a variety of age-appropriate toys and equipment. This violation was evidenced by Licensee Robins and Director Trujillo stated the puffy balls are no longer used in any activities with the infants. The center purchased a Safety First Choke Tube Tester. Deficiency cleared. Based on review of pictures and interviews, revealed the center was using small puffy balls which are not age appropriate for young infants which poses a potential risk to the health and safety of children in care.
