Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. Director will submit Criminal Background Transfer Request, along with signed and dated LIC508 (both pages) and a copy of her Driver's License to Licensing to associate her to the center, and Assistant Teacher Smith will not be working at the center until she is associated. Assistant Teacher Bailey Smith is cleared in the system, but center failed to submit a transfer request to associate her prior to working at the center. A $500 civil penalty was assessed.
