Based on interviews, documentation, Assistant Director stated moving forward, facility will ensure doctor notes that are received state child's clearances to return if child has an illness. A statement will be sent to LPA by POC due date. and the absence of a physician’s clearance, the facility failed to ensure that a child with a contagious illness was excluded from care and did not obtain a physician’s clearance. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's observation and interview the facility was out of ratio. There was 1 staff with 13 children Assistant Director stated staff have walkie talkies and they will give reminders to staff about communication. A staff meeting will also be held on October 14, 2024 and they will go over the importance of ratios. An Agenda and staff sign ins will be provide to LPA. for approximately 5 minutes. Deficient Practice Statement Based on observation, at 10:55 a.m., LPA observed the Emergency exit door, in the "Preschool Room," to not have a functioning door alarm. LPA pushed lever of the door open, and alarm did not sound. The Emergency door leads out into a shopping center and busy street. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. Pictures were taken. POC Due Date: 11/05/2021
