The director stated she posted a sing in the front entrance outside reminding parents to sign out and teacher will not release the child until the teacher knows the parent has sign out the child. The director stated she will write a statement with the new procedures and will email it to the LPA by the POC due date. Based on observation and record review LPA Nunez observed 8 signature slots were missing the time when they were not sign out on 9/6/2024, 9/5/2024, and 9/4/2024. The director stated they already put a fence that is higher. They purchase walkie talkie are in use to communicate with each other. And are in the process of hiring more teachers. The Director also stated she will write it in a written statement. Based on record review, staff interviews, and video footage. C1 eloped from the childcare due to lack of care and supervision This poses an immediate Health and Safety risk to the children in care.

LPA will also do an unannounced POC visit. Based on interview, S3 stated that A7 did not sign in C2 & C3 when being dropped off at the facility. Director stated she will write a written statement and send it to the LPA via email. Based on LPA observation and interview C1 was in the restroom without staff supervision. The Ratio was fixed on site during inspection. Director stated she is working on changing staff's schedules to ensure Teacher-Child Ratio is maintained. Owner is also hiring an additional teacher. Director will provided staff with training on Teacher-Child Ratio and email LPA sign-in sheet of training by POC Dute Date. Based LPA observation on 11/09/21 there was one staff providing care for 13 children. 2 Staff interviews and record review also show facility was out of ratio on 08/04/2021 and 08/18/2021 with 13 children present in the classroom. This poses an immediate safety risk for the children in care.

Bases on 2 Staff Interviews and Record Review, the Facility uses an unlicensed room to care for children. This poses an immediate safety risk for the children in care.

Director will also conduct a Staff Training to ensure staff is aware not to comingle the preschool and school age children. This poses an immediate risk to the safety of the children in care.
