The Director will train the infant staff on the Infant Care Food Service, the daily refrigerator checklist, the new feeding policy, and email staff signatures on or before 12/4/2025. Based on the information gathered from the self-reported unusual incident report, observations, and interviews with three staff, it was determined that S1 fed the wrong bottle of breast milk to Child #1 (C1) that belonged to Child #2 which poses a potential health, safety or personal rights risk to the persons in care. LPAs observations from 7:50 am until 8:07 am. At 7:50 LPA met with Lead Teacher Elizabeth Cuatecontzi who was providing care in Infant room. LPA viewed 5 infants up to 12 months old in care. LPA also viewed one aide providing care. At 7:58am LPA entered Director agreed to have staff change hours to include at least one more staff to begin at 7:00am and willl contact temporary agency to provide additional staff until further staff are hired.
