The Director will train the infant staff on the Infant Care Food Service, the daily refrigerator checklist, the new feeding policy, and email staff signatures on or before 12/4/2025. Based on the information gathered from the self-reported unusual incident report, observations, and interviews with three staff, it was determined that S1 fed the wrong bottle of breast milk to Child #1 (C1) that belonged to Child #2 which poses a potential health, safety or personal rights risk to the persons in care.
