    AD states that if a similar situation occurs, parents will be contacted immediately, child will be provided appropriate physical space until parent arrives. A mandatory meeting is being held on 05/20/23 which behavioral redirection will be discussed. LPA will receive topics and attendance sheet by 05/22/23. Additional training will be provided to all staff members by 06/16/2023. Based on video review and all interviews conducted, staff did not safely, physically redirect child from aggressive behaviors and move the child to a safe space.     Director will print out livescan form and send staff Gisela when she is available to complete the fingerprint clearance. Director will not allow staff Gisela to return to work until clearance is receieved and verified by LPA Castanon. Director will continue learning Guardian and audit facility rosters. Based on staff record review and interview with Director, there is no criminal clearance for Gisela Hernandez who has been employed at the facility since 06/01/2021.     Facility will implement hourly ratio checks on paper and perform headcounts. Director is going to plan ahead for additional staffing. At the beginning of the week Director will contact sister schools to provide additional staff. Director will email LPA ratio check sheet on 03/10/2023. Based on staff and parent interviews, staff have been out of ratio in their classroom on many occasions. This poses an immediate safety risk for the children in care.

    Director will request pest control company to visit facility more often. Director states management will perform classroom checks to make sure no food is left behind. Director will email copy of work order requesting pest control visits. Based on interviews, staff disclosed they have observed a rat, as well as, droppings in classroom and provided photos. This poses an potential health and safety risk for the children in care.
