The center will conduct a meeting with staff and retrain staff on the importance of active supervision at all times. Center has issued written warnings to all staff in classroom and provided LPA with signed acknowledment from staff and has implemented specific break and lunch times to each staff in classroom. Director will submit plan to LPA by end of business day April 17,2026. LPA also informed director additional drop in visits will continue to be necessary. Based on observations file review,and interviews, the center did not comply with the section cited above. The center did not provide sufficient supervision; child was left behind for approximately 5 minutes and went unnoticed twice. The center failed to provide adequate supervision to meet the needs of children in care this poses a potential health and safety risk for children in care. The center will conduct a meeting with staff and make it a reflective meeting and have the staff discuss what went wrong and how moving forward they can ensure this type of incident of absence of supervision does not continue to occur. Director will submit to LPA notes of meeting and a new plan of staff’s accountability when leaving and entering classroom with groups of children. Director will submit plan to LPA by end of business day February 27,2026. LPA also informed director additional drop in visits will be necessary. The center did not provide sufficient supervision; child was left in bathroom inside stall unattended while staff left with 4 children rather than the 5 she had taken.Child was discovered by other child entering with second group to use bathroom. The center has since developed a new plan of transitioing children from outdoor to indoor time through smaller groups. Director held a staff meeting and typed up notes during LPA visit and reminded staff of changes by having all staff read and sign ackowledging changes. Director wrote up plan in both English and Spanish and provided proof to LPA.

Immediately following the incident, the Site director held staff training to discuss policy regarding constant active supervision of day-care children, tracking strategies for classroom staff, and Children's Personal Rights. LPAs obtained these training materials and the signatures of all staff who attended the training. Based on interviews and file review, LPAs confirmed that child C1 was left alone inside the classroom for several minutes. This poses a potential health and safety risk to children in care.
