101216.3(a) · 101216.3: Teacher-Child Ratio: " (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance..." This requirement is not being met as evidence by Staff members stepped in and children were moved back to their classroom to be in compliance with appropriate teacher to child ratios.

Based on observation, interview, and record review, the facility did not comply with the section cited above as when LPA was conducting physical plant inspection, there was one staff with 22 children and then one staff with 15 children which poses a potential health, safety or personal rights risk to persons in care Facility must submit a written plan on how they will ensure that appropriate teacher to child ratios are met at all times, including on what steps to take when children need to use the restroom. Written plan must be submitted to LPA Tapia-Mandujano via email by 5/22/26.

101223(a)(2) · 101223: Personal Rights: " (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs." This requirement is not being met as evidence by LPA had a conversation with both Director and Site Director about what was observed.

Based on observation, the facility did not comply with the section cited above as LPA observed a child dozing off in a chair that is not appropriate equipment to sleep and a staff (S2) sitting on top of child's cot with a child in arms which is not safe. This poses a potential health, safety or personal rights risk to persons in care Facility will submit a written plan to acknowledge that incidents like these do not occur. Facility will also submit a written plan on how the facility will prevent incidents like these happening in the future. Written plan must be submitted to LPA Tapia-Mandujano via email by 5/22/26.
