The facility shall conduct a staff meeting regarding supervision requirements. A schedule shall be created to ensure that classrooms are sufficiently staffed to provide visual supervision of all children in care. A schedule shall be created and staff shall be assigned to provide coverage as needed. Based on observation, the licensee did not comply with the above by having 2 preschool children in a bathroom without the supervision of a teacher. This poses a potential risk to the health and safety of children in care. Preschool 1 shall have at least two staff members assigned at all times. The facility shall implement methods of communication between classrooms that include, but are not limited to, the use of two-way radios. A memo shall be signed by all staff and documentation of schedules shall be submitted via email by 10/20/2025. Licensee shall submit an updated morning staff schedule that accounts for more than 12 preschool age children being present in a classroom or outdoor space. A training shall also be conducted that covers ratio requirements, communication between staff regarding ratio, and assisting with ratio during the day. Based on site observations, interview, and record review, the facility did not comply with the above by having teachers supervise more than 12 children at a time without an aide or another teacher present. This poses a potential risk to the health, safety, or personal rights of persons in care. A training memo with meeting minutes detailing the content of the training shall be signed by all preschool staff and submitted to LPA by 8/15/2025. The updated morning schedule shall also be submitted to LPA by 8/15/2025.

Director stated prior to a child transitions from their assigned classroom to another classroom, their guardian and/or family will be notified in advance with written documentation. Director stated a written documentation form of parents’ acknowledgement of child’s classroom transition will be created. Director to provide staff training on effective communication with parents with written documentation form. . Based on observations, interviews and record review, the facility’s admission agreement states the involvement of a child’s family during child’s classroom transition. The facility moved a child from their assigned classroom to an older classroom without prior guardian consent, notice and/or involvement. Facility did not follow admission agreement or parent handbook. This poses a potential health, safety or personal rights risk to children in care. Evidence of this written documentation fomr will be submitted to LPA no later than 08/06/2021

Director stated all staff have received renewed training on “Name to Face” facility protocol with written documentation. Director has already provided personal renewed training and coaching. Evidence of staff meeting and T1’s personal training was provided to LPA during investigation. Based on interviews and record review, the facility self-reported a child was left unattended and unsupervised for 1-2 minutes.
