Licensee to submit health screening reports for Staff 1, Staff 3, and Staff 4 and TB results for Staff 1 and Staff 3 to LPA Billones via email provided no later than 5/22/2026. Based on staff records review, the Licensee did not comply with the section cited above in 3 staff records reviewed, which poses a potential health, safety, and personal rights risks to children in care.
