On 9/15/25, LPA Plumboy spoke with Beth Zygielbaum about the drawer with staff and children's records being accessible and not secure. Executive Director bought a locked file cabinet and is now utilizing it to safe keep children and staff records.
Based on observation, and tour of the facility, the office where the staff and children's files are stored was left open on 9/15/25, and the drawers of the file cabinet where the files were stored were not secure which poses a potential health and safety risk to children in care. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Director stated that she will contact executive director Beth to deep clean the playground. Director also stated that she will have the exterminator come to the facilty every other week and will provide recipts of visits. Director also agreed to send a proof of completion by end of day 12/31/2024
