Director Pattulo will submit a plan of action and includes dates of training to LPA by 05/01/2024. During LPA’s visit on 01/29/2024, LPA interviewed five (5) staff members. During interviews, 4 out of 5 staff stated they have witnessed S1 raise their voice at the children. LPA also observed S1 speak loud and direct to the children during their visit. Site Facilitator (SF) contacted District Office to locate background clearance documents for Staff Aida Gurrola.SF agreed Staff Aida cannot return to the facility until a Guardian system clearance and association is granted. SF will send LPA Guardian screenshot that Staff Aida is cleared and has been associated to the facility via email. Based on LPA's record review and interviews, there is no criminal clearance Staff Aida Gurrola who has been employed at the facility since 01/16/2004. Director and staff will watch personal rights and supervising children in childcare center videos. The Director will submit in writing proof of completion with the names and dates of all staff who completed the videos. Director will also submit a detailed email or written letter describing what plan of action the facility will address to go over the incident with staff and address compliance with children's personal rights by 11/1/2023. Based on interviews conducted with parents and staff as well as documentation received, a teacher handled a child/children in a rough manner. Teacher stated she understands the ratio but a child had to use the restroom. Teacher was left with 14 children as the assistant assisted a child to the restroom.
