This requirement has not been met as evidenced by Facility conducted a training on Friday 2/6/2026 with all staff during a Professional Development day focusing on Name to Face and Passport procedures that relate to transitions with children. Facility conducted a thorough investigation and has determined the appropriate corrective actions for staff to address non-compliance. LPA collected proof of training and corrective action during today's inspection. Based on Facility's self reported report received on 02/10/2026, C#1 was left unattended without staff supervision. This poses an immediate risk to the health, safety and personal rights of children in care.
