101238.2(g) · (g) The playground shall be enclosed by a fence to protect children and to keep them in the outdoor activity area.
The facility submitted a work order to fix the lock on the gate. The work order shows that the lock was fixed on 6/24/25. The facility also added a sign next to the gate that reads "Please close and latch gate" in English and Spanish. This deficiency is cleared. Based on interviews and record reviews, the fence in the outdoor activity area had a gate with a broken lock. This posed a potential health, safety, and personal rights risk to persons in care.
