101223(a)(1) · 101223(a)(1)- Personal Rights: (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons.

Licensee stated on 12/12/2025, Staff 2- S2 was spoken to and personal rights were reviewed with her, which stated children will have extra clothing at all times and no child will be sent outside without inappropriate clothing according to the weather. Based on interviews, the licensee did not comply with the section cited above by allowing Child 1- C1 to play outside in inappropriate weather in underwear's, which poses a potential health, safety, or personal rights risk to persons in care. In addition, Licensee stated an overview of personal rights and children's supervision shall be reviewed with all staff. Licensee will also review and submit a self-certification letter on section 101223(a)(1)- Personal Rights with all staff's signatures and to submit a copy to CCL by POC due date. Based on interviews, a child was left unattended outside for a brief period without supervision on 12/16/2025, which posed an immediate risk to the health, safety, and personal rights to children in care Upon receipt, Director shall post this report and provide copies of this licensing report, have LIC 9224 signed and kept on file, to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. According to AB 633, parents must be provided with this report which contains this Type A deficiency for the next 12 months & copy of the signed acknowledgment form must be kept in each child's file. 101227(a)(7)(A) · 101227 Food Services (a) In child care centers providing meals to children, the following shall apply (7) Modified diets prescribed by a child's physician as a medical necessity shall be provided. (A) The licensee shall obtain and follow instructions from the physician or dietitian on the preparation of the modified diet. Center director (CD) had purchased colored plates for children who has food allergy. CD will update C1's health care plan, also provide training to all staff, CD will submit POC to LPA by POC due date.

101229(a)(1) · Responsibility for Providing Care and Supervision - (a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Site Director Martha Morales submitted the Plan of Correction (POC) to LPA Morales during the exit interview. The POC is a written plan indicating what steps have been implemented to ensure that the preschool children are supervised at all times (inside and outside the classrooms). POC is cleared.

A child was found by a staff member unattended on the doorsteps of (LYNDALE CLASSROOM ONE) that leads to the outside playground whimpering/crying on April 8, 2022.
