By POC due date: 12/30/24, Licensee to submit a written plan on how they will ensure that children from different programs will be physically separated and off limit areas will not be used for daycare. Based on interviews, record review, and evidenced gathered, staff were commingling children from different programs and utilizing off limit areas for the preschool children which poses a potential health, safety or personal rights risk to children in care. Since this is a repeat violation within 12 months, an immediate civil penalty of $250 is hereby assessed.

101219(b)(4) · 101219 Admission Agreements (b) Admission agreements shall specify the following:(4) Modification conditions, including the requirement that the child's authorized representative be given at least 30-calendar-days prior written notice of any basic rate change.
Licensee shall provide parents with a 30 day written notice of any basic rate change and submit a copy to CCL by POC date 02/23/2024. Based on infromation obtained Director sent out a notice of increase child monthly rates to parents on February 8, 2024 effective March 4, 2024. 101170(e)(1) · Criminal Record Clearance) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record exemption as required by the Department.

The Licensee must provide copies of this report to parents/guardians of children in care at this facility and to parents/guardians of children newly enrolled at this facility during the next 12 months per the AB633 requirements. Based on observation, record reviews and interviews, Licensee did not comply with section cited above. S1 who was providing care to children enrolled did not obtain criminal record and child abuse index clearances prior to working in the facility. This poses an immediate threat to health and safety of children in care. The Licensee will submit a written statement to the Department by 06/30/23 indicating that they understand that all adult(s) must obtain the required criminal record and child abuse index clearances prior to working or residing in the home.
