101238(a) · Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. Licensee will submit a written plan to keep the classrooms and facility grounds clean, safe, sanitary and in good repair. Licensee will include plan of action to repair the chipped portion of the outside bathroom flooring by POC due date.

LPA observed LPA also observed a portion of laminated flooring located in the middle of the bathroom was chipped and needs repair to avoid trip hazard, which poses a potential health, safety or personal rights risk to persons in care
