101216.3(b)(1) · Teacher-Child Ratio (b) The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance. (1) A ratio of one fully qualified teacher (as specified in Section 101216.1(c) and one aide for every 18 children in attendance in a preschool program is allowed when the aide meets the qualifications specified in Section 101216.2(d).

Deficient Practice Statement Based on LPA's observation & record review, the licensee did not comply with the section cited above. A teacher and a teacher's aide were supervising 22 children in Room A8 which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 11/21/2025

A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. By the POC date: Facility will provide proof of repairs and retest results by 3/31/2023. Identified fixtures are not to be used until cleared for use after retesting. Bottled water is provided to the children and staff until the Identified fixtures are repaired. 101239(f)(1) · 101239(f)(1) Fixtures, Furniture, Equipment and Supplies: Solid waste shall be stored, located and disposed of in a manner that will not transmit communicable diseases or odors, create a nuisance, or provide a breeding place or food source for insects or rodents. All containers used for storage of solid wastes, including moveable bins, shall have a tight fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof. This requirement has not been met as evidenced by By the POC date the Licensee will replace the trash cans used to dispose of food with ones that have tight fitting covers. LIcensee will submit photos of the replaced trash cans to the CCLD. The LPA observed 5 trash cans that were missing tight fitting covers in rooms A-3 and A-4.
