    101216.3(a) · Teacher-Child Ratio - (a)There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance.

    Site Director states that she will submit a written plan by 10/16/25 to maintain the required ratio and capacity at all times. LPA observed a teacher (S3) alone in the Room 9 supervising 13 children. This posed an immediate risk to the health, safety, and personal rights of children in care. Because the facility repeated the same violation within 12 months, an immediate civil penalty of $1,000 is hereby assessed. An ongoing civil penalty of $100 per day will be assessed until the violation is corrected.
