A child was found unresponsive when their name was called, having vomited and urinated on themselves; staff only became aware of this when the light was turned on. The Director reviewed and retrained all staff on supervision, accountability, name-to-face, and passport procedures. Training materials were provided to LPA during the visit. In addition, the staff person involved in the incident has since resigned. The deficiency is cleared. Based on the interview and incident report, the staff failed to provide adequate supervision and adhere to the transitioning protocol, leaving C1 unaccounted for and alone in the bathroom.
