The facility has taken measure to investigate the incident and has conducted staff training regarding end of day protocols. Based on record review and interview, the Licensee did not comply with the above by leaving children without staff supervision in a classroom. This posed an immediate risk to the health or safety of children in care. The facility shall submit a meeting agenda detailing supervision requirements that are signed by all staff members present during the meeting. The facility shall also submit a plan for end of day staffing if there is a substitute. 101216.3(c) · 101216.3 - Teacher Child Ratio (c) Child development programs funded by the California Department of Education and operating under Title 5 of the California Code of Regulations are not required to meet the teacher-child ratios specified in (a) and (b) above. Title 5 staffing ratios shall apply in such centers. Per Title 5 regulations, the ratio for preschool age children is 1:8 adult-child ratio and 1:24 teacher-child ratio. Facility must maintain appropriate teacher to child ratio at any given time.

Based on record review and interview, the facility did not comply with this section cited above on May 19, 2026, which posed a potential heath, safety or personal rights risk to persons in care Facility will submit a new schedule for the program reflecting appropriate Teacher/Adult to Child ratios and a list of adults that may step in if needed to comply with ratio to LPA.
