The facility initiated corrective action with S1. S1's employment has since been terminated. The facility took appropriate action to address the situation. Based on record review and interview, the facility did not comply with the above due to a staff inappropriately handling a child, which posed a potential risk to the health, safety, or personal rights of persons in care.
