    The licensee should review and update record-keeping procedures to ensure that all children’s records remain complete, intact, and maintained for at least 3 years after termination of services. The licensee shall submit a written statement describing the corrective measures taken to prevent recurrence. Proof of correction is due by the date specified. Based on a review of records, the center did not comply with the regulation cited above. The center failed to keep certain child records after services were terminated. Failing to maintain children’s records for the required minimum of 3 years poses a potential health and safety risk to children in care.

    The center is required to submit an updated Plan of Operation and revised written policies to the Department for review. Once approved, the updated policies shall be provided to all enrolled families, and staff shall be instructed to follow the written procedures. The center must operate in accordance with the approved Plan of Operation at all times. Proof of correction is due by the date specified. Based on a review of records, the center did not comply with the above-cited regulation. Its current practices do not align with the written policies that remain in effect. It was found that certain procedures described in its written materials are no longer being implemented, and the written policies have not been agreed upon by an authorized representative to reflect these changes. Not operating in accordance with the approved Plan of Operation and written policies poses a potential health and safety risk to children in care.
