The center will review its admission agreement and provide refresher training for all staff members, covering topics such as a child's transition, a sample guardian and/or family notification letter, and effective communication with families. All training materials and staff members' signatures confirming their attendance must be submitted to the Licensing office by September 5, 2025. Based on interviews and record review, the center did not comply with the regulation cited above. The center’s admission agreement states that a child’s family should be involved during their transition to the classroom. However, the facility moved a child from their assigned classroom to a younger one without prior guardian consent, notice, or involvement. The center did not follow the admission agreement or the parent handbook. This poses a potential health, safety, or personal rights risk for children in care.

Facility will submit Child Napping Transition Plan and Staffing Plan / LIC500 to the Department (CDSS). Child napping transition plan submitted will include facility plan for classroom coverage during unanticipated shortages. In addition, facility will submit a written policy ensuring an alternative activity is provided to children who have completed their nap/ quiet rest. During interviews, it was reported, when adequate staffing is not available children are required to remain on napping cots during the scheduled rest period. This poses a potential health and safety risk to children in care. Proof of correction will be submitted to the Department by the due date: 7/7/2025. Documents will be submitted via email.
