101212(d) · 101212 reporting requirements (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.

The center must ensure that the facility is clean and free of flies, other insects, and rodents. The center will hire a professional pest control company. The center will send the work or and plan detailing what was done and a report of the findings from the company. Additionally the center will ensure the staff is constantly monitoring and ensuring the classrooms are clean and free of any dropping while children are present. The facility will ensure first thing in the morning the rooms are clean. The center will reach out to pest control within 24 hours. The center did not comply with the section cited above in both of the infant classrooms were observed to have had mice droppings. This poses and immediate risk to the health and safety of children in care.
