Facility self reported incident to department. Staff member involved in incident was placed on administrative leave. Director followed employee handbook's policies. Based on interviews and record reviewed, a staff member was observed to yell, hold and restrain a child. This poses a potential health, safety or personal rights risk to children in care. A staff professional development day is scheduled to be conducted March 27 2026. Director will discuss a child's personal rights with all staff. Proof of meeting and discussion to be submitted to LPA by POC due date.
