Faciltiy will meet with individual staff to set expectations, give first/final warning letter, conduct progressive training regarding how to deal with children with difficult behaviors, and follow up with parents. Based on interviews, facility did not ensure to notify the child's authorized representative of incident, which poses a potential Health, Safety, and Personal Rights risk to persons in care.
