Office meeting was previous conducted reminding licensee of the regulation to be present during operating hours. Licensee informed LPA that licensee plans to add a co-licensee to their license. LPA informed assistant present regulations will apply to both individuals who are licensees. Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee was not present during today’s visit. Per assistant, licensee will not be returning to facility until the end of March 2026, which poses an immediate health, safety or personal rights risk to children in care. Civil penalty was assessed today’s date. Licensee is planning to return to facility 3/28/2026. Case is currently under management review.
