Based on interviews conducted, the licensee did not comply with the section cited above, as facility staff used a bench, located next to the toilet in the children’s bathroom as a disciplinary tool, in the form of a time-out chair, which poses an immediate health, safety and personal rights risk to children in care.

Director stated the sign in sheet of those in attendance will be sent to the SDRO by 7/26/24. This is an amended version of the original report created on 07/16/24. Based on interviews and staff admissions, the licensee did not comply with the section cited above in that it was determined that on numerous unidentified dates staff left children unsupervised in the classroom, which poses an immediate health and safety risk to children in care.
