Director stated that staff member involved in incident was terminated and a staff meeting/training was held revising topics such as personal rights, potty training, and communication between coworkers and staff. Based on documentation and interviews, it was determined that that a staff member recorded and posted the child in an inappropriate manner and without permission. In a separate incident, a child was left without proper clothing, this poses an immediate health, safety or personal rights risk of children in care. Training was held on 4/14/2026 and all staff signed Agenda Notes. Proof of meeting notes were emailed to LPA Macias on 4/22/2026.

Director did a staff training on "Name to Face" training on 2/26/2025. LPA received a copy of Staff Meeting Agenda with staff signatures on 3/04/25.
