    Director stated that she met with all staff and reviewed video footage. Staff were reminded that they are to scann the room at all times and provide visual supervision. Director also stated that blankets or soft sheets are no longer allowed in the classroom. Based on staff interviews, and review of camera footage an infant was left on top of a boppy pillow on thefloor. A blanket was left above the infant’s head and due to lack of supervision, the infant was able to grab the blanket and dragged it over the infant’s head.

Based on staff interviews, and review of camera footage, it was observed that a parent walked into the classroom and found an infant's head covered with a blanket.
