Director stated she would submit the waiver and required documents for armed guards at the facility to LPA Lane by 2/14/25. Based on LPAs observation and interviews with Director and Licensee, the licensee did not comply with the section cited above in 2 armed guards on premises without applying for a waiver, which poses a potential health, safety or personal rights risk to persons in care.

Director stated she would send the construction documents, facility sketch and e-mail stating that the Department will be notified prior to construction/alterations of the physical plant and also upon completion. Director will send items requested above via e-mail to Keturah Lane by 2/14/25. Director stated that they added a wall and door between classrooms #122 and #121 (Multipurpose room) back in August 2023, but Licensing was not notified prior to the construction nor once completed, which is a potential health, safety and personal rights risk to persons in care.
