Correction have been made. Director has installed safety gates at both doors in classroom #3. Staff has been retrained with additional procedures regarding reporting incidents and families have been informed of incident and additional information to prevent this from happening again. Based on self reported incident C1, C2, C3 left the classroom and got through several doors and out of facility. This poses an immediate risk to the health, safety and personal rights of children in care. The licensee shall obtain specific instructions from the authorized representative regarding action to be taken. Incident reports shall be delivered and communicated to parents same day of the incident at pickup. Based upon interviews with Director, staff, complainant and parents, parent of C2 was not notified of the second bite upon pickup from the center. Other parents also stated that notification of injuries and behavior issues (biting) are not consistently communicated or received from the facility which is a potential health, safety and personal rights risk to children in care. Incident reports should be consistently written for both the biter and child that was bitten and records kept to know which child was bit and when. Director will provide a signed staff agenda dated by 2/23/24 and email to LPA Lane by EOB on 2/23/24.
