101430(a)(3)(C) · INFANT CARE ACTIVITIES An infant shall not be swaddled while in care.

Site Director stated that she will conduct a staff meeting specific to infants by November 28, 2023. She will send a copy of the agenda and staff signatures to acknowledge receipt of information. Based on the evidence gathered, an infant was observed to be swaddled and sleeping on a boppy pillow during nap. LPA confirmed during the inspection that the boppy pillow used by the infant was in the room.

101430(a)(3)(E) · INFANT CARE ACTIVITIES If an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible.

Site Director stated that she will conduct a staff meeting to discuss that no child is to sleep on boppy or be fed without supervision on the boppy. Ms. Low stated that she will remove the swings by end of business day, 11/27/23. Based on staff interview and photo received it was deteremined that infants were found to be sleeping in swings and boppy pillows for an undetermined amount of time before they were placed on cribs.

101416.5(b) · STAFF-INFANT RATIO. There shall be a ratio of one teacher for every four infants in attendance.

This deficiency was observed corrected today as one infant was moved back to Room # 7. Mr. Parker stated that he will designate a teacher/floater available as need arises. Based on LPAs observation, Room #4 was out of ratio when LPAs arrived with 9 infants and 2 staff (Antonio Parker and Noa Iovine).

All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record exemption as required by the Department. CORRECTED. Staff Abigail Hernandez and Rabi Josh Dorsch have obtained fingerprint clearance and are now associated to the facility.

Based on file review and observation by staff Tyra Block on 10/20/21, staff Abigail Hernandez was present in Room #7. Abigail Hernandez did not have a fingerprint clearance associated to the facility According to Mrs. Low, Rabi Josh Dorsch helped in the infant room to supervise napping infants on Oct. 11th and 12th.
