The director states that on 7/1/21 she conducted a staff meeting on supervision and required all staff to watch a video on Supervising Children in Childcare Settings. LPA's obtained a copy of a signed declaration/affidavit from staff members confirming that they watched the video at time of visit. Based on LPA's interviews with staff members and a witness the facility staff failed to maintain constant supervision and visual observation of a daycare child.
