Director had a safety meeting with all staff and provided the LPA with the itinerary and sign in sheet. The Director stated that she has implemented an additional head count check chart for staff and has also added a doorknob latch to the front door. The Director stated that she is also is adding a door chime and will add a safety gate in the front lobby area. Based on facility self report that C1 walked away from the group during a classroom transitioning and exited the facility. C1 exited the front door and was located 100 yards away in the apartment complex. This poses an immediate risk to the health and safety of the child.
