Director Gloria Hughes stated that she would provide a staff training regarding observation of the child and how to assess, monitor, and report injuries. Based upon interviews, video evidence, and medical records received, C1 was diagnosed with a head laceration. Director Gloria Hughes stated she will a submit UIR regarding the 10/17/23 incident along with a written statement that she understands and will abide by reporting requirements for future incidents. Following the injury incident involving the toy, Director Gloria Hughes acknowledged that the toy was not age appropriate and removed it from the classroom. This violation has been cleared. Based upon staff interviews and video evidence, C1 was injured by another child using a toy fishing rod and bait that was not age-appropriate for the classroom, which posed a potential health, safety and personal rights risk to children in care. We will meet with staff individually by 10/18/2021. A copy of topic covered and signatures will be submitted to the department no later than 10/18/2021. Based on LPA’s interviews, several staff admitted to not wearing their mask during circle time, inside the classroom. Classroom #12 was observed with children not wearing masks. This is an immediate hazard to the health of children due to the COVID-19 pandemic which is currently on the rise.

Licensee will provide updated enrollment agreement or addendum with correct hours of operation and a statement that all parents were provided the updated agreement by 8/13/21 via e-mail to LPA Lane.
