In the case of less serious injuries including, ...minor cuts, scratches and bites from other children requiring assessment and/or administration of first aid by staff, the licensee shall document the injury in the child's record and notify the child's authorized representative of the nature of the injury when the child is picked up from the center. Ms. Foster stated that she will conduct a training with staff to talk about reporting requirements to parents regarding injuries. She will have the training in writing and have staff sign it. Ms. Foster stated that she will have a maintenance man fill out the holes in the play yard, repair the fence (patch it) and will anchor the barricades properly. These corrections are due no later than 10/30/2021. Based on the interviews conducted with staff/parents and LPA's observation of the facility's physical plant, it was determined that the facility was not safe due to the latch to the fence was broken, fence had holes that a child can fit through, holes in the play area that poses a tripping hazard and the barricades that were not sturdy and not anchored properly. This poses a potential risk to the health, safety and personal rights of children in care.

Ms. Foster admitted that it was a poor judgement call and realized that it was something that she should've never done. She stated that she has signed up to complete 2 courses offered by NAEYC. She anticipates completion of these 2 courses by end of this week (9/24/21). Ms. Foster shall submit proof of course completion no later than 9/27/2021. Staff admitted to placing a waffle against a child's face in one incident and smearing cream cheese on a child's face in another incident. She stated that these act were done in a playful manner and was not done to tease them. Child became upset when staff smeared cream cheese on his face and cried.

This regulation was not met as evidenced by THIS WAS CORRECTED IMMEDIATELY. Ms. McDole removed the one school-age child and moved the one child to the school-age room. Another school-age child arrived during the inspection. Based on LPA's observation, upon arrival Nancy Diaz observed 7 children with staff Arianne Sanchez (6 preschool and 1 school-age child). Preschool and school-age children we co-mingled in the play area. Ms. McDole stated that school-age children will not be comingled with the preschool children.
