Based on observation, the facility front entry gate is broken and does not latch properly. LPA arrived at the facility and observed the front gate wide open. Mrs. Perusinghe will send a photo to show that a temporary latch was installed on the gate today, 8/18/2025.

This deficiency was corrected today. Ms. Sancha turned the feeder upside down to remove the water. Mrs. Perusinghe stated that she will obtain pebbles and place them on the bird feeders to avoide water retention. Based on observation there were 2 bird feeders filled with water located in the garden outside the classrooms. The garden is accessible to children. LPA observed children coming from the play area walking through the garden to their classrooms. Director and Staff stated that the gate was fixed by the handyman over the holiday weekend. LPA observed upon arrival that the gate was functioning properly and lock code was also working. LPA provided clearance letter at time of inspection. Based upon staff and Director interviews, the front gate was not working properly which is a potential health, safety and personal rights risk to children in care.
