Child returned to the supervision of staff and director spoke to all staff members immediately following the incident on 5/8/26. as observed by Facility Director Carlee Gosser provided LPA meeting agenda for the trainings facility conducted for staff on maintaining visual supervision at all time and implemented clear transition procedures and staff schedules. On 1/21/2026, staff failed to provide adequate supervision when a 3-year-old child was left unattended in the preschool play yard for approximately 1–2 minutes during a transition from outdoor play to the classroom. Licensee states that the sinks that tested in exceedance were made inoperable. The sink faucets have since been replaced, flushed and retested on 11/30/22. Sinks are now within the required limits.
