Site director was not available today. Ms. Pujeda stated that she will provide a copy of the site inspection report to Site Director Sarah Kimball. Ms. Pujeda stated that an all staff meeting was held on May 6th. A portion of the meeting talked about "Active Supervision". Based on staff and reporting party's interview, a staff did not prevent a daycare child to be harmed by another child. Staff was observed standing and watching the two children who were playing in the rug area. Staff did not intervene when the two children were fighting over a piece of toy.

The director states that she will conduct a staff meeting on 5/6/24 to review active supervision. Additionally, facility repairs were made so that doors self-close completely. The director will send LPA a copy of the meeting agenda and staff sign in sheet via email by 5/10/24. Based on interviews with the director, Staff #1 (S1) left child #1 (C1) without care and supervision, both inside and outside the facility, for several minutes on 4/19/24. This poses an immediate health, safety or personal rights risk to children in care.
