… Center director, Ms. Doakes, stated that the facility will seek to enhance the quantity and variety of toys and equipment for the preschool playground. She will provide documentation (invoices, photos) of purchases of such materials to the LPA Based upon interview and observation, it was determined that facility did not maintain sufficient variety and quantity of toys, equipment, and materials, which is a potential health, safety and personal rights risk to children in care. by the POC due date. Director states staff members were verbally spoken to on 9/9/25 and have been provided with instructions to ensure the supervision and safety of the children during outdoor activities. Based on interviews the facility did not ensure supervision of child C1, as child unaccounted for during outdoor activities, which posed a potential Health, Safety or Personal Rights risks to persons in care. Director wil conduct another meeting with staff on 10/22/25 regarding supervision and provide LPA with training materials a signed sheet to include all staff members present during the meeting.

Director stated there are several brick pillars in multiple classrooms that will all be covered with a bumber guard edge protectors by EOB 11/7/23. Director will send a picture of the correction via e-mail to LPA Lane by 11/7/23. Based upon LPA observation and facility unusual incident report - a brick pillar in the classroom was accessible and not covered causing injury to a child that fell and hit head on the ledge which is an immediate health, safety and personal rights risk to children in care. … Staff member was terminated upon review of the video footage of the incident. Facility reported the incident to Licensing following reporting requirements 101212(d)(1)(C). In addition, Director stated she will review Reporting Requirements Title 22: Section 101212 and e-mail a written statement to Licensing that she understands and will follow the reporting requirements. Based upon parent and Director interviews, a written incident report was not provided to parents, nor were parents advised of the incident verbally by facility which is a potential health, safety and personal rights risk to children in care. Director stated she would provide one week of updated sign in sheets (from preschool classrooms) by 3/17/23 via e-mail to LPA Lane with enough space for full legal signature of parents signing their child in and out as well as time of day (for drop off and pickup). Based upon LPA review of attendance sheets, Licensee did not ensure proper space for parents to sign full legal signature which is a potential health, safety and personal rights risk to children in care.

Director stated she would ensure that all child medications are communicated to all facility staff by posting name, picture of child and dates of medication where medications are kept and also in each classroom. Director will send an example of posting to LPA Lane via e-mail by 9/20/22.
