Based upon incident report received on 7/15/25 , video review, and interview with Director, children C1 and C2 were left unsupervised for approximately 30 seconds after exiting the classroom, which is a potential health, safety and personal rights risk to children in care. … Director stated (and LPA confirmed) that she met-one-one with the lead teacher of the classroom involved, as well as held a staff develop meeting on 2/17/25, in which indoor/outside supervision was reviewed. Director provided LPA with documentation from both meetings. The deficiency has been cleared. Based upon unusual incident report submitted by the facility and interviews with staff, C1 was left unattended outside the classroom and Directors Office, for approximately 2 minutes, which is a potential health, safety and personal rights risk to children in care. Correction was addressed day of incident on 12/17/24 . S1 accepted full responsibilty .Title 22 and company policy regarding safe handling of infant bottles was discussed. S1 reviewed and signed off on the training. Based on the fact that Child #1 was given Child #2's bottle to drink. This posed a potential risk to the health and safety of the child.
