Facility conducted remediation of lead on 6/30/22 and 7/1/22 and was cleared for occupancy by an approved agent of the Department of Public Health on 7/6/22. Based on interviews conducted and records review, the licensee did not ensure a safe environment as high levels of lead were detected in a classroom and restroom which posed a potential health and safety risk to children in care. Owner provided CCL with reports from lead testing showing the facility as cleared for occupancy.

Owner faxed transfer request to the SDRO during today's inspection. Based on records review and owner's admission owner did not ensure S3 had a criminal record clearance or exemption prior to working, residing, or volunteering in the licensed facility as required, which poses an immediate Health and Safety risk to the children in care.

Owner called the parents of 4 children to be picked up from the facility during the inspection. Owner will submit a plan of coorection by close of business 8/25/21 to ensure the facility operates within the conditions and limitations of the license. LPA observation and owner's admision owner did not ensure the condtions and limitations as sepcified on the license were observed as there were 24 children in attendance and the facility's capacilty is for 20 which poses an immediate Health and Safety risk to the children in care.
