Based upon incident report received on 8/27/25 and staff interviews, child C1 was left unsupervised for approximately 3 minutes in a classroom, which is a potential health, safety and personal rights risk to children in care. The Assistant director stated that she conducted a staff meeting to train all staff on proper supervision and transitions. The Assistant director has provided LPA with a copy of the meeting agenda and staff sign in sheets or staff acknowledgements that was taken on 4/11/23 . Based on information received by Assistant Director , Child #1 was in the hallway riding a scooter without supervision, for a period of approximately 4-5 minutes before being found by staff member and taken to the assistant director. This poses a potential health and safety risk to children in care.

101212(d) · (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.
Assistant Director will provide an unusual incident report for other incident that occurred in March where child was possibly left unattended. She will ensure that in the future incidents are reported on the duty line (619)767-2248 within 24 hours and will submit an incident report by email to SDIncidentReports@dss.ca.gov or by Fax at (619 767-2203 within 7 days of occurring. Assistant Director will also provide written statement explaining that she will submit future reports per the required time frames. This requirement was not met as evidenced by the department did not receive an incident call within 24 hours and di not recieve a report within 7 days .

Director stated that the water fountain at has been covered and a do not drink signage has been placed on the water fountain. There is a zip lock bag with tape placed on the fountain to prevent children from drinking the water. Based on record review, the facility did not comply with the directive above, as one out of eight drinking or food prep outlets tested with an Action Level Exceedance (ALE). Facility has not been using the outdoor drinking fountain since March/2019 due to COVID. This poses a potential Health and Safety risk to children in care. Facility has replaced the water fixture and has been flushing the drinking fountain. Facility is awaiting for the water tester to return to test the new drinking fountain at the playground.
