Director states she will submit in writing facilty's step to inform CCLD of reporting requirements of any incidents or outbreak that may occure by 6/30/22. Based on Director and Licensee’s admission, epidemic outbreak was not report to CCLD reported within 24 hours or in writing within 7 days from the initial occurrence. Based submission of multiple Unusual Incident Reports on 1/20/22, it was determined between 1/6/22 -1/19/22. Director did not report an epidemic outbreak to CCLD within 7 days. This poses a potential risk to children in care. This is an amended version of the orginal created on 5/10/22.

Based on multiple interviews conducted, it was revealed that on multiple occasion office personnel are drinking on the premises during facility's operating hour Director stated the updated Alcohol, Tobacco and substance abuse policy will be discussed at staff meeting that is scheduled for 9/15/21. Director stated a staff attendance roster, meeting agenda and updated policy will be submitted to the department no later than 9/17/21. Based on interviews conducted and records reviewed, the licensee did not ensure the center operated in accordance with Alcohol, Tobacco, Substance Abuse Policy, on multiple occasions when facility staff consumed alcoholic beverages on the premises, during working hours. This poses a potential Health and Safety risk to clients in care. This is an amended version of the 9099-D, "This is an amended version of the originial report created on 8/17/21.

Director stated that she will verify with LPA prior to staff member being present in the classroom. Also, Director will verify staff members eligibility monthly by contacting and verifying with assigned LPA. Based on the director's admission, the director did not ensure that S1 had a criminal record clearance or exemption prior to working, residing, or volunteering in the licensed facility as required, which poses an immediate Health and Safety risk to the children in care.
