… Area manager stated that the immediate correction is that staff member (S1) has not been present with children at the program since 12/13/22 and is no longer working for the company since 3/6/23. All staff members and newly hired staff will undergo health and safety training including personal rights on 3/6/23. Area manager stated she will provide written agenda of the meeting along with staff attendance sign in sheet via e-mail to LPA Lane by 3/7/23. E-mail: Keturah.Lane@dss.ca.gov Based upon documents received and interviews conducted with staff, children and parents, it is determined that staff member (S1) handled a child in a rough manner which is an immediate health, safety and personal rights risk to children in care.

Site Supercisor states staff will be at the parent table at all times to ensure parents are signing out correctly and will provide written plan to LPA by POC due date of 11/23/22. Based on interviews and record review a child was not signed out of the program at pick up on at least one occasion on 9/7/22. This poses a potential health and safety risk to children in care.

The director threw away the expired milk at the time of the inspection. The director states that she will review the contents of the refrigerator to dispose of old/expired food/beverages weekly. Based on LPA's observation the facility had two containers of expired milk in the facility refrigerator. This poses a potential health and safety risk to children in care.
