  records review by LPA Via phone, the Licensee stated she will conduct the fifteen minute checks of all infants ages 0 through 24 months and will document the fifteen minute checks by 07/30/2021. The proof of documentation shall be emailed to the LPA by 07/30/2021. Marie.Hernandez@dss.ca.gov The Licensee has an infant age one and half years old but there is no documentation of the infant safe sleep fifteen minute checks for infants 0 through 24 months. This poses a potential safety risk to children in care. The appeal rights were discussed and provided to the Licensee via phone and a copy was provided to the Adult Helper, Elizabeth Gil-Cervantes.
