Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above. Staff D.M.'s fingerprint clearance is not associated to the facility. Mrs. Finney provided proof to LPA that she walked in the documents to the department in October, however records did not show that staff DM's fingerprint was associated. 101238.2(e) · OUTDOOR ACTIVITY SPACE As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls.
Mrs. Finney stated that the climbing structure is off-limits to children until sufficient cushioning material is installed. She anticipates completion of installation of cushioning by July 17, 2024. In the meantime, staff will install a yellow "caution" tape and not allow children not to use the climbing structure.
